Just depends on what I'm in the mood for. Sometimes I want to read something else by that author or something with a similar theme. Or sometimes if I've been reading the same kind of book for a while then I'll shoot to the other side of the literary universe. How do you choose?
I have lists upon lists to go to. And I also see books regularly at work that look interesting(duh...lol), and they frequently get routed back to me once they are in the collection. I have about 10 books checked out right now. About 5 others on my personal 'to read' shelf. And I'm reading three right now.
Even if you don't buy from them, amazon.com is a great site. Once you find a book of interest you will have many links to books that others liked if they liked the one you're interested in. I also can spend hours in Barnes & Noble.
I stand in front of our bookshelves and scan them, picking up things that look interesting. Gradually, I end up paring it down to something I actually want to read, or re-read, as the case may be.
What a great question! I'm like you -- just trying to plow through my massive TBR pile. Unless something new comes out and pushes to the fore, like the new Paul McCartney bio I have just about finished. Then it's back to the pile.

I pick the next book in the stack. I've got soooooooo many piled up.

It's usually an impulse. I read or hear about a book that sounds interesting, and I go pick it up.

Right now the only books that I am reading are on my syllabus.
When I am free to pick the books that I read, I usually look for authors tat I have read before.
